You must read the Athletic Handbook in its entirety before signing this form. The handbook includes the pricing of season passes and other questions you may have. When reading the online handbook ignore the signature places under the Code of Conduct for Parents and Student-Athletes. Those signatures will be placed on this web form and submitted for the Code of Conduct.
