DECLARATION OF MORAL INTEGRITY FORM
Curtis Baptist School is a religious, nonprofit organization representing Jesus Christ throughout the local community. Curtis Baptist School requires its employees and volunteers to be born-again Christians, living their lives as Christian role models (Rom. 10:9 – 10, 1 Tim. 4:12, Luke 6:40). Employees/volunteers will conduct themselves in a way that will not raise questions regarding their Christian testimonies. A Christian lifestyle should reflect the biblical perspective of integrity, appropriate personal and family relationships, business conduct, and moral behavior. An employee/volunteer is expected to demonstrate a teachable spirit, an ability to share a love for others, a willingness to live contentedly under authority, and a commitment to follow the Matthew 18 principle when an issue arises with fellow employees/volunteers or administration.
Our school expects all of its employees/volunteers who have unsupervised access to children, to model the same Christian values and lifestyle that it seeks to inculcate in its students. As an applicant for a ministry position as an employee/volunteer at this school, I must recognize, understand and agree to live by the Christian moral standards of Curtis Baptist School.
I declare that as a follower of Christ, I am not engaging in and commit to not engaging in inappropriate sexual conduct. Inappropriate conduct includes, but is not limited to, such behaviors as the following: heterosexual activity outside of marriage (e.g., premarital sex, cohabitation, extramarital sex), homosexual activity, sexual harassment, use of (including the viewing of) pornographic material or websites to include the inappropriate use of social media, and sexual abuse or improprieties toward minors as defined by Scripture and federal or state law.
Additionally, if employed at Curtis Baptist School, knowing that my life is a constant testimony to those to whom I am called to lead, I agree to abstain from drinking alcohol, using tobacco, vaping of any kind, and/or inappropriate texting/messaging.
School employees/volunteers will maintain a lifestyle based on biblical standards of conduct. Failure to do so may result in a reprimand or, in some cases, dismissal from employment/volunteering. It is the goal of Curtis Baptist School that each employee will have a lifestyle in which “Christ may have the preeminence” (Col. 1:18 NKJV).
I declare that the above statement is factual and true. My signature below indicates that I meet the moral integrity standards and Christian role model lifestyle requirements of Curtis Baptist School.

Statement of Faith
We believe the Bible, both the Old and the New Testaments, to be inspired by God in its original languages and to be our final rule of faith and practice (II Timothy 3:16-17).
We believe in the deity, virgin birth, vicarious death, burial, and the bodily resurrection of Jesus Christ (Colossians 2:19, Matthew 1:23, Matthew 27, Luke 23).
We believe that Jesus Christ ascended into heaven and sits at the right hand of God the Father as our advocate (Mark 16:19).
We believe in the personal Second Coming of Jesus Christ for His saints (Revelation 3:11, 1Thessalonians 5:1-10).
We believe in a literal creation, the fall of man, and that all men are sinners by birth (Genesis 1-3).
We believe that each person is accountable to God for his sin and must accept Jesus Christ’s sacrificial substitutionary death as payment for his sin ( Romans 8:1-17, 3:23).
We believe that salvation is by God’s grace alone and not by man’s works or goodness (Romans 3:24-26, Romans 8:1-17)
We believe in a literal heaven and hell (Matthew 10:28, Luke 12:5)
We believe that God gives us eternal life through Jesus Christ. The true believer is secure in their salvation for eternity (John 10:10, Ephesians 1:13).
I fully agree with the Statement of Faith as written without reservations.
